The VA Ann Arbor healthcare system was among facilities mentioned in a letter to President Obama from Carolyn Lerner of the U.S. Office of Special Counsel.
A whistle blower reported workers were practicing unsafe and unsanitary work practices, and untrained workers were improperly handling instruments.
The Ann Arbor facility has not issued a comment.
